Puffy eyes in the morning are often fluid. While you lie flat all night, fluid collects in the thin tissue under and around your eyes, and it drains away once you are upright. Salt, alcohol, allergies and short sleep make it worse; age and genetics add eye bags that do not drain. A cool compress while sitting up, a raised pillow tonight and less salt at dinner are the steps Mayo Clinic and the American Academy of Ophthalmology both give.

Why do I wake up with puffy eyes?

Mayo Clinic lists fluid retention "especially upon waking or after a salty meal" as a cause of puffy eyes, and explains that the space below the eyes can gather fluid. Mayo Clinic The American Academy of Ophthalmology says the same: fluid retention, "often after waking up or after eating salty food". AAO Cleveland Clinic describes the swelling as fluid that builds up in the thin layers of tissue around the eyes and eyelids. Cleveland Clinic

Why the morning? Gravity. A review of eyelid swelling written for eye surgeons notes that it is often much worse in the morning after a long time reclined, or after a lot of salt the night before. Eyelid Edema review, PMC A study of 40 adults measured a related thing with ultrasound, skin thickness: skin on three areas of the face, including the corner of the eye, was thicker in the morning and thinner by the afternoon, while skin on the legs did the opposite. The authors suggest that gravity moves fluid from the face to the legs during the day. Br J Dermatol 2001

What makes waking up with puffy eyes worse:

  • Salt. Salt makes your body hold water, which can show as puffiness around the eyes. Cleveland Clinic
  • Alcohol. AAO lists drinking alcohol among the factors that add to eye bags. AAO For the full morning-after picture, see does alcohol make your face puffy.
  • Allergies. Feather pillows, pets on the bed and skin care you put on before sleep can all cause an allergic response, says AAO, which also advises against rubbing your eyes. AAO
  • Short sleep. Mayo Clinic lists lack of sleep as a cause. Mayo Clinic The research is mixed (see the quick answers below).
  • Lying flat. The authors of a 2009 ophthalmology article say that lying horizontal, on your back or your front, lets fluid drain into the lower lid. That is their expert view. Review of Ophthalmology
  • Age and genetics. With age, the tissue that supports the eyes weakens and fat moves into the lower lids, and under-eye bags can run in families. Mayo Clinic This is the part that does not drain away by lunch.

How to get rid of puffy eyes in the morning: a 5-minute routine

Order matters more than products. Get upright first, cool the area second, then help the fluid along.

0:00 — Get up and stay up. Do the whole routine sitting or standing. Mayo Clinic and AAO both tell you to use a cool compress while sitting up. Mayo Clinic AAO Cleveland Clinic says to lie down for it instead. Cleveland Clinic We follow Mayo and AAO, because gravity is what drains the fluid once you are up; that is our reasoning, not a trial result.

0:30 — Cool compress, a few minutes (we suggest 2 to 3). Wet a clean washcloth with cool water. Hold it on the skin under and around your eyes with light pressure. Mayo Clinic says "for a few minutes". Mayo Clinic An ice pack or frozen vegetables also work if you wrap them in a towel to protect your skin. Cleveland Clinic Never put ice straight on the eyelid, and do not press on the eyeball.

3:00 — Light under-eye strokes, 1 minute. Rest your ring finger on the bone under your eye and stroke from the inner corner to the outer corner, 5 times, with the lightest touch you can. 4:00 — If your eyes itch or water, treat the allergy. Do not rub. AAO suggests an over-the-counter antihistamine when an allergic response starts. AAO Cleveland Clinic adds that antihistamine eye drops help only if you have allergies. Cleveland Clinic Ask a pharmacist which one suits you.

How long do puffy eyes last in the morning?

No medical body gives a number for ordinary morning puffiness. What the sources do say:

  • Cleveland Clinic, on eyelid swelling in general: usually an annoyance that goes away on its own within a day. Cleveland Clinic
  • An ophthalmology trade article (2009): from clinical experience, morning lid swelling can last up to six hours. It is one group's experience, not a measurement. Review of Ophthalmology
  • The 2001 ultrasound study: face skin was measurably thinner in the afternoon than in the morning. Br J Dermatol 2001

So a fair rule: mild puffiness in both eyes that is clearly better by midday is the usual pattern. Swelling that lasts longer than 24 to 48 hours is a reason to see an eye care professional, says ophthalmologist Annapurna Singh at Cleveland Clinic. Cleveland Clinic

Puffy eyes or eye bags? A morning-afternoon check

Both can sit under the same eye. AAO lists fluid pooling and fat shifting forward into the lower lids as separate causes. AAO No medical source gives a home test to tell them apart, so here is the one we use, based on the morning-to-afternoon change above:

  1. Take a photo within 15 minutes of waking: same window, same distance, face straight on, relaxed.
  2. Take a second photo in the same spot at about 3 p.m.
  3. Compare the lower lids side by side.
  • Much smaller in the afternoon: mostly fluid. (The study above measured skin thickness, not eyelid volume, so this check is our reasoning, not a tested method.) The routine and the night plan below are the right tools.
  • About the same: mostly structure (fat and skin). Cold and massage will not change it much. Mayo Clinic says bags under the eyes are usually a cosmetic concern that does not need medical treatment; if they bother you, it lists laser resurfacing, chemical peels, fillers and eyelid surgery as options to discuss with a health care provider.   • Getting worse over weeks, not years: see a doctor. A surgeons' review says that a change in eyelid appearance that has not developed slowly over years deserves a careful check. Eyelid Edema review, PMC

This is a personal check, not a diagnosis. Light and camera distance change how puffy a lid looks, which is why the conditions need to match.

How to prevent puffy eyes tonight

  • Raise your head a little. Add an extra pillow or prop up the head of the bed a few inches. This helps stop fluid from collecting around your eyes as you sleep. Mayo Clinic
  • Drink less right before bed, and eat less salt. Mayo Clinic and AAO both advise cutting down on fluids before bedtime and limiting salt. Mayo Clinic AAO
  • Go easy on alcohol, for the reasons above.
  • Take off eye makeup and skip heavy eye cream at night if you suspect it. Mascara, eyeshadow and eye creams can contain fragrances, dyes and preservatives that irritate the skin or cause an allergic reaction. Cleveland Clinic
  • Check the bed. Feather pillows and pets sleeping on the bed are on AAO's list of allergy triggers. AAO
  • Protect your sleep window. Mayo Clinic suggests 7 to 9 hours for most adults. Mayo Clinic

Puffy eyes every morning? Check these causes

If your eyes are puffy almost every morning for weeks, the usual salt-and-sleep advice is probably not the whole story. Work through this list:

  1. Your eye products. Eczema around the eyes is often a contact allergy to products. In two German dermatology clinic groups (88 and 2,035 patients sent to skin clinics with red, itchy eczema around the eyes), allergic contact dermatitis was the main cause, and everyday products such as face cream and eyeshadow were a leading source. Br J Dermatol 2008 Cleveland Clinic suggests stopping eye makeup and eye creams for at least a couple of weeks to see if they are the cause.   2. Your bedroom. Dust mites are a known trigger of allergic conjunctivitis. StatPearls Add feather pillows and pets, and your bed can be the source. If antihistamines help, allergy is likely part of it; an allergist can test.
  3. A CPAP mask. In a case report (two patients, not a study of CPAP users in general), two people using a full-face CPAP mask had swelling around the eyes that was worse in the morning and faded during the day; it improved after they switched to a nasal-pillow mask. Case report, 2015 Ask your sleep clinic before you change masks.
  4. Medicines. Many common medicines can cause eyelid swelling, including corticosteroids, aspirin and other anti-inflammatory painkillers, and hormone treatments. Eyelid Edema review, PMC Do not stop a prescribed medicine on your own; ask your doctor.
  5. Swelling elsewhere. Puffy eyelids together with swollen legs or ankles, weight gain from fluid, or foamy urine can be signs of a kidney problem called nephrotic syndrome. NIDDK Swelling around the eyes with bulging eyes can be thyroid eye disease. Cleveland Clinic Both need a doctor, not a routine.

Why is only one eye puffy in the morning?

A review for eye surgeons notes that how you sleep can make swelling uneven between the two eyes. Eyelid Edema review, PMC If it is mild, painless and fading through the morning, sleep position may be one reason. Swelling of one eyelid that is red, painful or tender is different: it can be a stye or an infection of the skin around the eye (periorbital cellulitis), which is usually one-sided. Popular fixes: what the evidence says

  • Cold compress: the first home step from Mayo Clinic and AAO. Mayo Clinic We found no trial of cold on ordinary morning puffiness. After eyelid surgery, one small randomized trial found cooling eased pain but did not reduce swelling, Plast Reconstr Surg 2015 while a 2025 review of surgery trials found that cold compresses gave short-term relief. Orbit 2025 Cheap, safe, quick: worth doing, with modest expectations.
  • Caffeine eye creams and chilled tea bags: Cleveland Clinic says caffeine products can "sometimes" improve how eye bags look. Cleveland Clinic We found no randomized trial of caffeine alone on puffiness; the studies we found test mixed creams on dark circles. J Cosmet Dermatol 2024 A chilled tea bag is mostly a cold compress.
  • Cucumber slices and cold spoons: cold objects. Johns Hopkins lists them alongside ice packs. Johns Hopkins Medicine No evidence they beat a cool washcloth.
  • Drinking a big glass of water on waking: brand pages recommend it; we found no source showing it drains your eyes. The medical advice is about fewer fluids before bed.
  • Hemorrhoid cream: avoid it. Cleveland Clinic warns against using it around the eyes. Cleveland Clinic In one reported case, a woman who applied hemorrhoid ointment to her eyelids for several weeks developed a dilated pupil. J Emerg Med 2012

Get urgent medical help if your eyelid is red, hot, painful, tender or blistered, if your eyesight changes, if the pain is in the eye itself, if light hurts your eyes, or if you have a high temperature or feel generally unwell. NHS (UK) guidance Call 911 (or your local emergency number) for sudden swelling of the lips, mouth, tongue or throat, or any trouble breathing or swallowing. NHS: angioedema

Quick answers

Is it normal to wake up with puffy eyes every day?

Mild puffiness that is gone or much smaller by midday is common. Mayo Clinic says bags under the eyes are usually a cosmetic concern and rarely a sign of a serious condition. Mayo Clinic If it is new, getting worse, or comes with the warning signs in the box above, get it checked.

Does lack of sleep cause puffy eyes?

Probably a little, after a lot of lost sleep. In a 2013 study, 40 people rated photos of 10 volunteers; after 31 hours without sleep the faces were rated as having more swollen eyes. Sleep 2013 A 2019 study of 181 people found no difference in skin colour, eye openness, mouth shape or dark circles after one night without sleep; it did not measure swelling. J Sleep Res 2019 Both studies tested a full night or more of lost sleep. Short sleep is worth fixing anyway, but it is rarely the only cause.

Can sleep apnea cause puffy eyes?

We found no source linking sleep apnea to ordinary morning puffiness. Sleep apnea is linked to a different eyelid problem, floppy eyelid syndrome. Clin Exp Ophthalmol 2022 If you snore loudly or wake unrefreshed, see a doctor about the sleep itself. If you already use CPAP, check the mask (see above).

Are puffy eyes a sign of kidney problems?

Mayo Clinic lists kidney disease among the medical conditions behind puffiness under the eyes. Mayo Clinic With nephrotic syndrome, a kidney condition, puffy eyelids come with other signs: swelling in the legs, ankles or belly, foamy urine and weight gain from fluid. NIDDK If you have those signs, see a doctor.

Should I cut salt or drink more water?

Both, at different times. Cut salt and drink less in the last hour or two before bed, but drink normally during the day: AAO lists not drinking enough water as a factor too. AAO Drinking extra water to "flush" puffiness has no evidence behind it.

The short version

Morning puffy eyes are often fluid that settled while you lay flat. Get upright, use a cool compress for a few minutes while sitting up, and add light under-eye strokes if you like. Tonight, raise your pillow, eat less salt, drink less before bed and go easy on alcohol. If it happens every morning, check your eye products, bedroom and medicines.
